By submitting this form, I acknowledge and agree that all Chasing Tail events are subject to review, scheduling availability, and location scouting. Submission of this form does not constitute a binding agreement or guarantee that an event will be scheduled at my venue.

I understand that if my venue does not sell food, I must allow approved outside food vendors or food trucks to operate during the event.

I acknowledge that Chasing Tail events involve dogs and people in a shared environment and that inherent risks may exist. I agree that the venue owner/operator assumes all responsibility and liability for any damage, injury, or incident occurring on the premises during the course of the event, except as otherwise required by law.

I agree to cooperate with Chasing Tail staff regarding event setup, safety protocols, capacity limits, and operational guidelines to ensure a safe and successful event.

I acknowledge that Chasing Tail will attempt to accommodate my request, pending availability and location suitability.

I confirm that my venue can safely accommodate at least 50 people and 50 dogs.

If my venue does not sell food, I agree to allow outside food vendors or food trucks during the event.

I accept responsibility and liability for any damage or injury occurring on the premises during the event, except as required by law.
